NEW STUDENT APPLICATION CHECK LIST:
_____ Completed Application for Admission form
_____ Non-refundable $50 Application Fee
_____ Request for Records form submitted to the student’s previous school*
_____ Teacher/Principal Recommendation form submitted to the student’s previous school*
_____ Certified copy of the student’s birth certificate (students must be five years of age by September 1st to enroll in kindergarten or six years of age by September 1st to enroll in 1st grade of the year they are enrolling)
_____ Copy of the student’s up-to-date Immunization Record – Georgia State Form #3231**
_____ Current hearing and vision test results from the pediatrician or health department for all kindergarten and 1st grade students
_____ If applicable, copies of custody and/or guardianship papers
*Transcripts and Teacher Recommendation forms must be received in the Eastside Christian School Office directly from the student’s previous school, either through the mail or in a sealed envelope that has been certified by the student’s previous school.
** See your pediatrician or the health department for the immunization form required by Georgia Law which must be on file for each student. Additional vaccination requirements for all rising 6th grade student includes a second MMR and Varicella.
Testing will be scheduled as needed upon completion of the Check List. Receipt of the above documents does not imply that a binding contractual agreement has been made.
